FILLED FORM PATTERN-
PM MODI on 9 May 2015 will launch another Accident Insurance Scheme by the name
of PRADHAN MANTRI SURAKSHA BIMA YOJANA. In this Bima Yojana you would be
offered a coverage of Rs 2 lakh in case of Accidental death or Disability on account of
accident. This Scheme will be only for one year, after one year you have to renew the
scheme. This Bima Yojana will be offered by Public Sector General Insurance
Companies (PSGICs) and other General Insurance company. Any Indian whose age is
in between 18 to 70 year having bank account can enroll in this Bima Yojana.
The premium amount which is of 12 Rs per year will be auto-debited from your bank
