Lesion Complexity
• Heterogeneity of the composition, distribution,
and location of atherosclerotic plaque within the
native coronary artery results in unique patterns
of stenosis morphology in patients with CAD
• These patterns have been used to identify risk
factors for procedural outcome and complications
after PCI and to assess the risk for recurrent
events in patients who present with an acute
coronary syndrome
Data obtained from 846 lesions undergoing qualitative angiographic analysis at the Washington Hospital Center
Angiographic Core Laboratory
• The Society for Cardiovascular Angiography
and Interventions (SCAI) risk score used an
ordinal ranking of two composite criteria
(vessel patency and complex morphology) to
classify lesions into four groups:
– Non type C patent
– Type C patent
– Non type C occluded
– Type C occluded
• The Mayo Clinic Risk Score added the integer
scores for the presence of eight morphologic
variables and provided a better risk
stratification than the ACC/AHA lesion
classification for the prediction of
cardiovascular complications
• whereas the ACC/AHA lesion classification was
a better system for identification of
angiographic success

Lesion Length
• Lesion length may be measured by a number of methods
– measurement of the “shoulder-to-shoulder” extent of
atherosclerosis narrowed by more than 20%
– quantification of the lesion length more than 50% narrowed
– estimation of the distance between the proximal and distal
angiographically “normal” segment
• last method is used most commonly in clinical practice and
provides a longer length than more quantitative methods
• Diffuse (>20 mm) lesions are associated with reduced
procedural success with drug-eluting stents, primarily
related to large degrees of late lumen loss and more
extensive underlying atherosclerosis
Ostial Location
• Ostial lesions are defined as those arising within 3 mm of origin of
the vessel or branch
• can be further characterized into aorto-ostial and non–aorto-ostial
• Aorto-ostial lesions are often fibrocalcific and rigid, requiring
additional ablative devices, such as rotational atherectomy in the
presence of extensive calcification, to obtain adequate stent
expansion
• Positioning of the proximal portion of the stent in the aorto-ostial
location so that no more than 1 mm of stent extends into the aorta
requires meticulous care
• Ostial stenoses that do not involve the aorta may also be more
elastic than nonostial lesions and require the same principles for
treatment as bifurcation lesions
Angulated Lesions
• Vessel angulation should be measured in the
most unforeshortened projection at the site of
maximum stenosis using a length of curvature
that approximates the balloon length
• Angioplasty of angulated lesions increased the
risk for:
– Dissections
– Inability to deliver the stent
– Subsequent straightening of the vessel that may
predispose to late stent fracture
Bifurcation Lesions.
• The optimal strategic approach for bifurcation
lesions remains controversial, and the risk for
side branch occlusion during PCI relates to the
relative size of the parent and branch vessel,
the location of the disease in the parent
vessel, and the stenosis severity in the origin
of the side branch. In general, placement of
one stent is preferable to stent placement in
both the parent vessel and side branch
Degenerated Saphenous Vein Grafts
• A serial angiographic study in patients undergoing
coronary bypass surgery showed that 25% of SVGs
occlude within the first year after coronary bypass
surgery
• Although a drug-eluting stent may reduce the
recurrence rate due to restenosis, only embolic
protection devices have reduced procedural
complications
• The extent of graft degeneration and estimated volume
of plaque in the target lesion are independent
correlates of increased 30-day major adverse cardiac
event rates
Lesion Calcification
• Coronary artery calcium is an important marker
for coronary atherosclerosis.
• Conventional angiography is moderately sensitive
for the detection of extensive lesion calcification
but is less sensitive for detection of milder
degrees of lesion calcification
• Severely calcified lesions tend to be more rigid
and undilatable than noncalcified lesions, and
rotational atherectomy may be useful before
stenting to ensure stent delivery and complete
stent expansion
Thrombus
• Although conventional angiography is a relatively
insensitive method for detection of coronary thrombus
• Presence is associated with a higher risk of procedural
complications, primarily relating to embolization of
thrombotic debris into the distal circulation
• Large, intracoronary thrombi may be treated with a
combination of pharmacologic agents (e.g.,
glycoprotein IIb/IIIa inhibitors) and mechanical devices
(e.g., passive aspiration and rheolytic thrombectomy
TOTAL OCCLUSION
• Total coronary occlusion is identified as an abrupt
termination of the epicardial vessel
• anterograde and retrograde collaterals may be present and
are helpful in quantifying the length of the totally occluded
segment
• Success in passage of a coronary guidewire across the
occlusion depends on the occlusion duration and on certain
lesion morphologic features, such as bridging collaterals,
occlusion length of more than 15 mm, and absence of a
“nipple” to guide advancement of the guidewire
• presence of a total occlusion remains one of the major
reasons for referral of patients for coronary bypass surgery
CORONARY PERFUSION
• Perfusion distal to a coronary stenosis can
occur anterograde by means of the native
vessel, retrograde through collaterals, or
through a coronary bypass graft
• The rate of anterograde coronary flow is
influenced by both the severity and
complexity of the stenosis and the status of
the microvasculature

Coronary Collateral Circulation
• Networks of small anastomotic branches interconnect the
major coronary arteries and serve as precursors for the
collateral circulation that maintains myocardial perfusion
despite the development of severe proximal atherosclerotic
narrowings
• Collateral channels may not be seen in patients with
normal or mildly diseased coronary arteries because of
their small (<200 μm) caliber, but as CAD progresses and
becomes more severe (>90% stenosis), a pressure gradient
is generated between the anastomotic channels and the
distal vessel that is hypoperfused
• The trans-stenosis pressure gradient facilitates blood flow
through the anastomotic channels, which progressively
dilate and eventually become visible as collateral vessels

• The visible collateral channels arise from
– contralateral coronary artery
– ipsilateral coronary artery through intracoronary
collateral channels
– through “bridging” channels that have a
serpiginous course from the proximal coronary
artery to the coronary artery distal to the
occlusion
• These collaterals may provide up to 50% of
anterograde coronary flow in chronic total
occlusions and may allow the development of a
“protected” region of myocardial perfusion that
does not develop ischemia during enhanced
myocardial oxygen demands
• Recruitment of collateral channels may occur
quickly in patients who develop an acute ST-
segment elevation myocardial infarction caused
by a sudden thrombotic occlusion
• Other factors that affect collateral
development are patency of the arteries
supplying the collateral and the size and
vascular resistance of the segment distal to
the stenosis
• Grading of flow of collaterals can be classified
by the Rentrop criteria
– Rentrop grade 0 (no filling)
– Rentrop grade 1 (small side branches filled)
– Rentrop grade 2 (partial epicardial filling of the
occluded artery)
– Rentrop grade 3 (complete epicardial filling of the
occluded artery)

Lesion Length • Lesion length may be measured by a number of methods – measurement of the “shoulder-to-shoulder” extent of atherosclerosis narrowed by more than 20% – quantification of the lesion length more than 50% narrowed – estimation of the distance between the proximal and distal angiographically “normal” segment • last method is used most commonly in clinical practice and provides a longer length than more quantitative methods • Diffuse (>20 mm) lesions are associated with reduced procedural success with drug-eluting stents, primarily related to large degrees of late lumen loss and more extensive underlying atherosclerosis

Ostial Location • Ostial lesions are defined as those arising within 3 mm of origin of the vessel or branch • can be further characterized into aorto-ostial and non–aorto-ostial • Aorto-ostial lesions are often fibrocalcific and rigid, requiring additional ablative devices, such as rotational atherectomy in the presence of extensive calcification, to obtain adequate stent expansion • Positioning of the proximal portion of the stent in the aorto-ostial location so that no more than 1 mm of stent extends into the aorta requires meticulous care • Ostial stenoses that do not involve the aorta may also be more elastic than nonostial lesions and require the same principles for treatment as bifurcation lesions

Angulated Lesions • Vessel angulation should be measured in the most unforeshortened projection at the site of maximum stenosis using a length of curvature that approximates the balloon length • Angioplasty of angulated lesions increased the risk for: – Dissections – Inability to deliver the stent – Subsequent straightening of the vessel that may predispose to late stent fracture

Bifurcation Lesions. • The optimal strategic approach for bifurcation lesions remains controversial, and the risk for side branch occlusion during PCI relates to the relative size of the parent and branch vessel, the location of the disease in the parent vessel, and the stenosis severity in the origin of the side branch. In general, placement of one stent is preferable to stent placement in both the parent vessel and side branch

Degenerated Saphenous Vein Grafts • A serial angiographic study in patients undergoing coronary bypass surgery showed that 25% of SVGs occlude within the first year after coronary bypass surgery • Although a drug-eluting stent may reduce the recurrence rate due to restenosis, only embolic protection devices have reduced procedural complications • The extent of graft degeneration and estimated volume of plaque in the target lesion are independent correlates of increased 30-day major adverse cardiac event rates

Lesion Calcification • Coronary artery calcium is an important marker for coronary atherosclerosis. • Conventional angiography is moderately sensitive for the detection of extensive lesion calcification but is less sensitive for detection of milder degrees of lesion calcification • Severely calcified lesions tend to be more rigid and undilatable than noncalcified lesions, and rotational atherectomy may be useful before stenting to ensure stent delivery and complete stent expansion

Thrombus • Although conventional angiography is a relatively insensitive method for detection of coronary thrombus • Presence is associated with a higher risk of procedural complications, primarily relating to embolization of thrombotic debris into the distal circulation • Large, intracoronary thrombi may be treated with a combination of pharmacologic agents (e.g., glycoprotein IIb/IIIa inhibitors) and mechanical devices (e.g., passive aspiration and rheolytic thrombectomy

Coronary Collateral Circulation • Networks of small anastomotic branches interconnect the major coronary arteries and serve as precursors for the collateral circulation that maintains myocardial perfusion despite the development of severe proximal atherosclerotic narrowings • Collateral channels may not be seen in patients with normal or mildly diseased coronary arteries because of their small (<200 μm) caliber, but as CAD progresses and becomes more severe (>90% stenosis), a pressure gradient is generated between the anastomotic channels and the distal vessel that is hypoperfused • The trans-stenosis pressure gradient facilitates blood flow through the anastomotic channels, which progressively dilate and eventually become visible as collateral vessels
