Nurse Leadership & Clinical Reasoning.
Nurse Leadership & Clinical Reasoning. In the modern era, there are few professions that do
not to some extent rely on data. Stockbrokers rely on market data to advise clients on
financial matters. Meteorologists rely on weather data to forecast weather conditions, while
realtors rely on data to advise on the purchase and sale of property. In these and other
cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s
body of knowledge. Of course, the nursing profession also relies heavily on data. The field of
nursing informatics aims to make sure nurses have access to the appropriate date to solve
healthcare problems, make decisions in the interest of patients, and add to
knowledge.Nurse Leadership & Clinical Reasoning. In this DiscussionORDER A
PLAGIARISM-FREE PAPER HERE, you will consider a scenario that would benefit from
access to data and how such access could facilitate both problem-solving and knowledge
formation. To Prepare: •Reflect on the concepts of informatics and knowledge work as
presented in the Resources. •Consider a hypothetical scenario based on your own
healthcare practice or organization that would require or benefit from the access/collection
and application of data. Your scenario may involve a patient, staff, or management problem
or gap. By Day 3 of Week 1 Post a description of the focus of your scenario. Describe the
data that could be used and how the data might be collected and accessed. What knowledge
might be derived from that data? How would a nurse leader use clinical reasoning and
judgment in the formation of knowledge from this experience?Nurse Leadership & Clinical
Reasoning.Concepts of informaticsIn recent years, the health sector has embraced
digitization of processes, data capture, and storage. This trend is attributable to the need to
make data-driven decisions and leverage on past events to learn and plan for the future as
well as improve healthcare service delivery. A key aspect of quality service delivery is
getting it right and offering services at the right time.Let’s observe a scenario whereby
patients are unhappy with the quality of service received in a health facility. They are often
misdiagnosed and have to wait for long periods before they are treated. M. Goossen and P.
Weber 2017 discuss the possibility of recording patient information and reusing the facts to
build on the knowledge and improve on service delivery. Health facilities, therefore, need to
implement effective data collection and storage system. At the same time, the nurse leader
is responsible for ensuring that all staff members are proficient and aware of the need for
accurate data capture and proper storage (D. McGonigle and K. Mastrian 2017). Nurse
Leadership & Clinical Reasoning. To resolve the problem of misdiagnosing patients, health
practitioners can refer to information in the central repository to compare symptoms and
prescriptions. (Nagle, Sermeus & Junger, 2017). Nurses can access medical information
accumulated over time and assess trends to build on their medical knowledge. On the
operational side, a queue management system is implemented to track service duration.
Data collected from this system aids in identifying inefficient processes and forms a basis
for business improvement processes. Therefore, we can conclude that informatics is an
integral part of the present-day nursing practice and health care in general. Data repository
enables analysis and study of medical data to advance the field. Health leadership also
carries the responsibility of ensuring all staff are well informed and are compliant with the
set service guidelines.Nurse Leadership & Clinical Reasoning.Technology is ever-changing
within healthcare settings. Nursing informatics is essential within clinical practice and
requires ongoing use of various databases in order to provide adequate care (Nagle,
Sermeus, & Junger, 2017). Understanding and utilizing technology such as electronic patient
records can increase patient satisfaction, decrease medication errors, and promote safety
for patients and family members. It is imperative not only as a registered nurse but as a
future advanced practice nurse that health safety and quality is utilized when accessing
patients’ charts and providing care. Electronic health records (EHR) are a systems utalized
by health care facilities in order to access patient data, collaborate with interdisciplinary
staff, and provide overall safe care. EHR’s is a database also utilized to prevent errors by
“improving the efficiency of the trigger tool technique” (Burlison, et al., 2018). Trigger tools
can reduce an adverse reaction event by providing alerts before medication
administration.Nurse Leadership & Clinical Reasoning. A scenario that occurs often is
medication errors by nurses. At times, not appropriately using databases such as the
Electronic record database while administrating medications during rounds can lead to
safety risks. A nurse who utilizes electronic health records without appropriately
identifying patients before administering medications increases patient harm. While
conducting the morning medication rounds, the nurse fails to use appropriate actions to
identify a patient, which results in patients receiving an incorrect medication. The nurse
failed to use the tools to identify the right patient appropriately.Nurse Leadership & Clinical
Reasoning. A nurse leader would use clinical judgment and clinical reasoning by
implementing actions that would reduce patient errors and potential harm. As nurse
leaders, continuing to educate nurses and staff on accessing the EHR system and using
patient identifiers to reduce harm or patient death. Essential data such as the patient
names, medical record numbers, and date of birth is the initial step in verifying the
appropriate patient. Understanding how to improve patient quality and safety is essential to
healthcare systems (McGonigle & Mastrian, 2018, p. 16). Reducing patient errors can occur
when is wisdom is utilized. “wisdom uses knowledge and experience to heighten common
sense” (McGonigle & Mastrian, 2018, p. 12). Nursing roles often require an understanding of
patient safety and using systems such as electronic health records.Nurse Leadership &
Clinical Reasoning.Informatics is progressively evolving and is vital to the healthcare
profession. Technology is an invaluable tool in patient care implemented in the use of
electronic records, administering medications, patient portals, mobile devices, and
communications, wearables, and other digital technologies (Booth, 2016). Informatics is
now providently recognized as essential to the healthcare industry that the specialization
has been formalized in the nursing profession, known as nursing informatics. Nursing
informatics streamlines safe, cost-effective, quality care.ScenarioThe Corona Virus has
altered the healthcare delivery system and expedited telemedicine as a deliver care at a
distance solution. Telehealth triage, diagnosis, and delivery of routine care have become a
standard way to maintain distance and safety during the pandemic. Providers have adapted
practices to determine if appointments are needed face to face or by a telehealth
visit. Informatics plays a pivotal role in gathering information, documenting in electronic
health records, communications, data sharing, and reporting to the Centers for Disease
Control (CDC). The data that is gathered can then be used to implement healthcare
strategies to improve outcomes.Nurse Leadership & Clinical Reasoning.Nurse Leadership &
Clinical ReasoningNurse leaders must adhere to standardized screening, make clinical
judgments to guide patients, and follow CDC guidance to reduce exposure. Enforcing CDC’s
protocols in healthcare facilities will help reduce the risk of transmission and protect
healthcare personnel (CDC, 2020). Though the nurses utilizing telehealth services may not
be informatics specialists, their perspective of design and utility are essential to the
development process (Nagle, Sermeus& Junger, 2017).In conclusion, in this scenario or any
clinical setting, nurse leaders are accountable to appropriately implement, train, and
monitor the efficient use of technology to effectively access data and knowledge (McGonigle
& Mastrian, 2017). Adhering to nursing terminology and technology informatic guidelines is
imperative for efficient integration. Since COVID 19, the expansion of telemedicine has
transitioned the method of delivering patient care. This is now the progressive safe
approach to connect with patients, engage in discussions, collaborate with colleagues, and
exchange knowledge and information.Nurse Leadership & Clinical Reasoning.Concepts of
informaticsLike many other sectors, the health sector has embraced digitization of
processes, data capture, and storage. Traditional manual systems that involved paperwork
lacked a centralized data repository that is accessible at an instance. It is also almost
impossible to analyze data in paper form. Digitization has propelled practices away from
the ineffective manual methods, and they can now store and access data with a click of a
button. In this light, health centers have created data banks that hold relevant health care
information. Nurses, health care providers, and administrators then use this data to analyze,
guide process improvements and improve the medical field.Nurse Leadership & Clinical
Reasoning.Goossen and P. Weber 2017 discuss the possibility of recording patient
information and reusing the facts to advance the medical field. For instance, patient vitals
and symptoms recorded are analyzed; the similarity in trends and correlation is
extrapolated to diagnose illnesses correctly. In the wake of new strings of infections like the
COVID – 19, health practitioners and researchers will leverage on collected information to
test and eliminate treatment procedures that were not effective from past trials (L. Nagle et
al., .2017). This process has resulted in the discovery of a treatment for malaria, flu, to
mention a few. Nurses can access medical records accumulated over time and assess trends
to build on their medical knowledge (D. McGonigle and K. Mastrian 2017).Nurse Leadership
& Clinical Reasoning. Therefore, we can conclude that informatics is an integral part of the
present-day nursing practice and health care in general. Data repository enables analysis
and study of medical data to advance the field. We have seen learning material, vaccines,
and treatments invented to remedy illnesses that killed many people in the past. On the
operational side, the need to collect information on the duration spent in health facilities
and the time lag between treatment process steps has enabled the development of systems
like the queue management system. Data collected from this system aids in identifying
inefficient processes and forms a basis for business improvement processes.Nurse
Leadership & Clinical Reasoning.

Nurse Leadership Clinical.docx

  • 1. Nurse Leadership & Clinical Reasoning. Nurse Leadership & Clinical Reasoning. In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge. Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge.Nurse Leadership & Clinical Reasoning. In this DiscussionORDER A PLAGIARISM-FREE PAPER HERE, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation. To Prepare: •Reflect on the concepts of informatics and knowledge work as presented in the Resources. •Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap. By Day 3 of Week 1 Post a description of the focus of your scenario. Describe the data that could be used and how the data might be collected and accessed. What knowledge might be derived from that data? How would a nurse leader use clinical reasoning and judgment in the formation of knowledge from this experience?Nurse Leadership & Clinical Reasoning.Concepts of informaticsIn recent years, the health sector has embraced digitization of processes, data capture, and storage. This trend is attributable to the need to make data-driven decisions and leverage on past events to learn and plan for the future as well as improve healthcare service delivery. A key aspect of quality service delivery is getting it right and offering services at the right time.Let’s observe a scenario whereby patients are unhappy with the quality of service received in a health facility. They are often misdiagnosed and have to wait for long periods before they are treated. M. Goossen and P. Weber 2017 discuss the possibility of recording patient information and reusing the facts to build on the knowledge and improve on service delivery. Health facilities, therefore, need to implement effective data collection and storage system. At the same time, the nurse leader is responsible for ensuring that all staff members are proficient and aware of the need for accurate data capture and proper storage (D. McGonigle and K. Mastrian 2017). Nurse Leadership & Clinical Reasoning. To resolve the problem of misdiagnosing patients, health practitioners can refer to information in the central repository to compare symptoms and

On the operational side, a queue management system is implemented to track service duration. Data collected from this system aids in identifying inefficient processes and forms a basis for business improvement processes. Therefore, we can conclude that informatics is an integral part of the present-day nursing practice and health care in general. Data repository enables analysis and study of medical data to advance the field. Health leadership also carries the responsibility of ensuring all staff are well informed and are compliant with the set service guidelines.Nurse Leadership & Clinical Reasoning.Technology is ever-changing within healthcare settings. Nursing informatics is essential within clinical practice and requires ongoing use of various databases in order to provide adequate care (Nagle, Sermeus, & Junger, 2017). Understanding and utilizing technology such as electronic patient records can increase patient satisfaction, decrease medication errors, and promote safety for patients and family members. It is imperative not only as a registered nurse but as a future advanced practice nurse that health safety and quality is utilized when accessing patients’ charts and providing care. Electronic health records (EHR) are a systems utalized by health care facilities in order to access patient data, collaborate with interdisciplinary staff, and provide overall safe care. EHR’s is a database also utilized to prevent errors by “improving the efficiency of the trigger tool technique” (Burlison, et al., 2018). Trigger tools can reduce an adverse reaction event by providing alerts before medication administration.Nurse Leadership & Clinical Reasoning. A scenario that occurs often is medication errors by nurses. At times, not appropriately using databases such as the Electronic record database while administrating medications during rounds can lead to safety risks. A nurse who utilizes electronic health records without appropriately identifying patients before administering medications increases patient harm. While conducting the morning medication rounds, the nurse fails to use appropriate actions to identify a patient, which results in patients receiving an incorrect medication. The nurse failed to use the tools to identify the right patient appropriately.Nurse Leadership & Clinical Reasoning. A nurse leader would use clinical judgment and clinical reasoning by implementing actions that would reduce patient errors and potential harm. As nurse leaders, continuing to educate nurses and staff on accessing the EHR system and using patient identifiers to reduce harm or patient death. Essential data such as the patient names, medical record numbers, and date of birth is the initial step in verifying the appropriate patient. Understanding how to improve patient quality and safety is essential to healthcare systems (McGonigle & Mastrian, 2018, p. 16). Reducing patient errors can occur when is wisdom is utilized. “wisdom uses knowledge and experience to heighten common sense” (McGonigle & Mastrian, 2018, p. 12). Nursing roles often require an understanding of patient safety and using systems such as electronic health records.Nurse Leadership & Clinical Reasoning.Informatics is progressively evolving and is vital to the healthcare profession. Technology is an invaluable tool in patient care implemented in the use of electronic records, administering medications, patient portals, mobile devices, and communications, wearables, and other digital technologies (Booth, 2016). Informatics is now providently recognized as essential to the healthcare industry that the specialization
  • 3. has been formalized in the nursing profession, known as nursing informatics. Nursing informatics streamlines safe, cost-effective, quality care.ScenarioThe Corona Virus has altered the healthcare delivery system and expedited telemedicine as a deliver care at a distance solution. Telehealth triage, diagnosis, and delivery of routine care have become a standard way to maintain distance and safety during the pandemic. Providers have adapted practices to determine if appointments are needed face to face or by a telehealth visit. Informatics plays a pivotal role in gathering information, documenting in electronic health records, communications, data sharing, and reporting to the Centers for Disease Control (CDC). The data that is gathered can then be used to implement healthcare strategies to improve outcomes.Nurse Leadership & Clinical Reasoning.Nurse Leadership & Clinical ReasoningNurse leaders must adhere to standardized screening, make clinical judgments to guide patients, and follow CDC guidance to reduce exposure. Enforcing CDC’s protocols in healthcare facilities will help reduce the risk of transmission and protect healthcare personnel (CDC, 2020). Though the nurses utilizing telehealth services may not be informatics specialists, their perspective of design and utility are essential to the development process (Nagle, Sermeus& Junger, 2017).In conclusion, in this scenario or any clinical setting, nurse leaders are accountable to appropriately implement, train, and monitor the efficient use of technology to effectively access data and knowledge (McGonigle & Mastrian, 2017). Adhering to nursing terminology and technology informatic guidelines is imperative for efficient integration. Since COVID 19, the expansion of telemedicine has transitioned the method of delivering patient care. This is now the progressive safe approach to connect with patients, engage in discussions, collaborate with colleagues, and exchange knowledge and information.Nurse Leadership & Clinical Reasoning.Concepts of informaticsLike many other sectors, the health sector has embraced digitization of processes, data capture, and storage. Traditional manual systems that involved paperwork lacked a centralized data repository that is accessible at an instance. It is also almost impossible to analyze data in paper form. Digitization has propelled practices away from the ineffective manual methods, and they can now store and access data with a click of a button. In this light, health centers have created data banks that hold relevant health care information. Nurses, health care providers, and administrators then use this data to analyze, guide process improvements and improve the medical field.Nurse Leadership & Clinical Reasoning.Goossen and P. Weber 2017 discuss the possibility of recording patient information and reusing the facts to advance the medical field. For instance, patient vitals and symptoms recorded are analyzed; the similarity in trends and correlation is extrapolated to diagnose illnesses correctly. In the wake of new strings of infections like the COVID – 19, health practitioners and researchers will leverage on collected information to test and eliminate treatment procedures that were not effective from past trials (L. Nagle et al., .2017). This process has resulted in the discovery of a treatment for malaria, flu, to mention a few. Nurses can access medical records accumulated over time and assess trends to build on their medical knowledge (D. McGonigle and K. Mastrian 2017).Nurse Leadership & Clinical Reasoning. Therefore, we can conclude that informatics is an integral part of the present-day nursing practice and health care in general. Data repository enables analysis and study of medical data to advance the field. We have seen learning material, vaccines,

On the operational side, the need to collect information on the duration spent in health facilities and the time lag between treatment process steps has enabled the development of systems like the queue management system. Data collected from this system aids in identifying inefficient processes and forms a basis for business improvement processes.Nurse Leadership & Clinical Reasoning.
