In addition to the consistency
of the primary care provider
assignment, the Medical
Home model focuses on five
key aspects of care:

Care Teams)
5. Comprehensive Care
Patient-Centered Medical Home
• When visiting primary care, students will
be assigned to a primary care provider
who is part of a care team.
• What does this mean for students?
Students will see members from the
same care team with each appointment.
• This approach encourages
communication and partnerships
between individual patients, their
primary care physicians, other team
members (counselors, psychiatrists,
nutritionists, etc.), and when
appropriate, the student’s family.
Received accreditation as a Medical Home in 2014 and
re-affirmed in August 2017 by the
American Association Ambulatory Health Care (AAAHC)

We empower students, faculty and staff to be self-directed, well- informed consumers of health care.   • 9. Patient-Centered Medical Home In addition to the consistency of the primary care provider assignment, the Medical Home model focuses on five key aspects of care: 1. Quality and Safety 2. Accessibility 3. Patient-Centered: patient/provider relationship 4. Continuity (Coordinated Care Teams) 5. Comprehensive Care
  • 10. Patient-Centered Medical Home • When visiting primary care, students will be assigned to a primary care provider who is part of a care team. • What does this mean for students? Students will see members from the same care team with each appointment. • This approach encourages communication and partnerships between individual patients, their primary care physicians, other team members (counselors, psychiatrists, nutritionists, etc.), and when appropriate, the student’s family. Received accreditation as a Medical Home in 2014 and re-affirmed in August 2017 by the American Association Ambulatory Health Care (AAAHC)
