Objectives To provide participants with a guide for best practice in handling errors. To educate participants on how to: ,[object Object]
Understand the 5 whys method to determine root cause
Plan and implement actions
Verify effectiveness of actions implemented
Recognize the benefits of corrective & preventive actions.,[object Object]
Used Terminology Error = Nonconformance ,[object Object]
CAs = Corrective Actions
PAs = Preventive Actions
RCA = Root Cause Analysis
QA = Quality Assurance
GDP = Good Documentation Practice,[object Object]
Report the error
Take immediate action to contain the error
Investigate the error
Find the likely cause of the error.
Take CAs to correct the error
Take PAs to prevent recurrence of the error.,[object Object]
Write with a pen,NEVER a pencil, AVOID whiteouts. ,[object Object],[object Object]
If it is NOT documented it is NOT done
Documentation is to be complete and legible.
Data should be recorded in a manner that may be clearly understood by an outside auditor.,[object Object]
Do NOT mention names of patients  or employees.  ,[object Object]
Avoid words such as, I, He, She, poorly, inadequately, haphazardly, carelessness and complacency.
Do not pinpoint fingers at anyone.,[object Object]
Ensure that OERs are signed by your supervisor and not just left behind.,[object Object]
Take immediate action: Examples ,[object Object]
Remove XY & segregate from other thermometers.
No documented procedure addressing AB requirement.
Write a procedure addressing AB requirement.
Inadequate instructions in a procedure.
Update the procedure accordingly.,[object Object]
Root Cause  After the problem/event is contained, the causes must be identified to prevent recurrence.  ,[object Object]
Contributing causes also called Intermediate causes are those that may not have sufficient power to result in the event taking place. 
Root Cause is simply the last logical cause in the chain.,[object Object]
