Abstract To assess the patient satisfaction level in emergency department of a level 1Trauma Centre in India. Shallu Chauhan, Dr.Deepak AgrawaL. JPN Apex Trauma Centre, All India Institute of Medical Sciences, New Delhi-110029, India Introduction Patient satisfaction is an important indicator of the quality of care and service delivery in the emergency department (ED). The objective of this study was to evaluate patient satisfaction level in the E.D. of a level 1 Trauma Centre,AIIMS,New Delhi.To determine the effects of actual waiting time,perception of waiting time,information delivery and expressive quality on patient satisfaction. Methods This study was carried out for 2 months during all shifts mostly for those patients who triaged as green.We made two groups:1) control group{ not explained anything to the patient} and 2) test group{patient explained for time management & treatment}. Patients/relatives were asked to complete the questionnaire prior to discharge. For the first month, eight questions were based on descripitve information were distributed to the control group { questions including explanation of procedures to the patient,communication of staffs,problems faced by patient/relatives, and overall patient satisfaction level}.Then, following second month another study questionnaire included 11 questions based on a Likert scale concerning waiting time{ie,overall time management,waiting for X-ray or C.T,scan,review by doctor, for discharge & treatment},promptness & behaviour of staff and cleaniness of hospital given to the test group. Observation Ninety patients who attended our ED were included in this study.The perception that waiting times for placebo injection & T/t were less than expected was associated with a positive overall satisfaction rating for the ED encounter[p is 0.033] as compared to actual waiting time.Actual waiting time were not predictive of overall patient satisfaction. The highest satisfaction rates were observed in cleaniness of hospital in both the groups and most of them rated it as very good. For overall treatment, in control group 34% rated as poor & fair and 67% rated as good and very good,whereas in test group only 22% rated as poor and fair but78% rated as very good and excellent.At the same time,both the groups were rated as good for overall time management but they were not satisfy with the time taken by doctor to review the reports and 33% rated as fair in control group and 22% rated as fair in test goup.The assigned waiting time for particular physician to review a report was 60minutes but average time taken to consult a particular physician was >60mins which mostly occur in control group.The overall satisfaction rate was dependent on the mean waiting time. The highest waiting time for a low rate of satisfaction of patient was 180minutes and for very good level of satisfaction was just 15minutes. Team Nursing: Team nursing is a collaborative care delivery model that involves a team of healthcare professionals working together to provide comprehensive care to a group of patients. The team typically includes registered nurses, licensed practical nurses, and nursing assistants, among others. A registered nurse usually leads the team and coordinates care, delegating tasks to team members based on their abilities and scope of practice. This model encourages teamwork, shared decision-making, and a holistic approach to patient care. By leveraging the collective skills and knowledge of the team, team nursing aims to provide high-quality care while maintaining efficiency. Primary Nursing: Primary nursing is a patient-centered care delivery model in which a registered nurse takes full responsibility for coordinating and providing care to a specific group of patients. The primary nurse develops a therapeutic relationship with the patients, assesses their needs, creates individualized care plans, and provides direct care throughout their healthcare journey. This model emphasizes continuity of care and places a strong emphasis on the nurse-patient relationship. By having a dedicated primary nurse, patients benefit from personalized attention, improved communication, and a sense of continuity in their care. Case Management: Case management is a care delivery model that focuses on coordinating healthcare services for patients with complex medical conditions or multiple healthcare needs. A case manager, often a registered nurse, plays a key role in this model by working closely with the patient, their family, and other healthcare professionals to ensure seamless transitions between healthcare settings and to optimize resource utilization. The case manager assesses the patient's needs, develops and implements care plans, advocates for the patient, coordinates appointments and services, and collaborates with various healthcare providers. The goal is to provide comprehensive and efficient care while maximizing patient outcomes and resource allocation. Each of these care delivery models has its own strengths and weaknesses, and the choice of model depends on factors such as the healthcare setting, patient population, and available resources.

PROFICIENCY REPORT FROM Ms. O PRACTICE Ms. O is a registered nurse on the inpatient psychiatric unit for (35-37) acute / rapid stabilization of (MH/BS), assigned to evening tour of duty, coed patients with diversified backgrounds at different developmental stages. The patient’s clinical presentation typically presents as acute depression, bipolar, schizophrenia, anxiety, substance abuse problems, and PTSD including psychosocial issues. Ms. O functions as charge nurse continue to demonstrate leadership, greater accountability, and knowledge in the delivery and application of her nursing practice, critical thinking, planning, prioritizing, collaboration, and coordination of skills while guiding, and directing (13-15) staffs in the provision of direct quality nursing care with minimal supervision. Practice: Applies the nursing process to systems or processes at the unit/team/work group level to improve care. Demonstrates leadership by Involving others in improving care. Ms. O applied nursing process to the provision of patient care in order to improve care outcome. Ms. O demonstrated using nursing process in the following ways: completes and implements initial assessment, reevaluation with follow-up based on the patients presenting individual clinical presentation, provides education upon assessment of patient/family’s educational needs, and team for a safe discharge plan. She participates to address the overall holistic function and maintenance of the Veteran’s physical, emotional, and social environment toward the goals of achieving physical survival, self-determination, and autonomous community functioning because of (assessment, planning, monitoring, advocacy, and implementation) contributing to the Veterans full potential. Ms. Oke assist to safely discharge approximately (25-30) patients per week and this discharge performance contributes to helping reduce the length of stay (LOS) as indicated from fy 2014, from (12.3-7.56) days per utilization management. During shift reports as Charge nurse, assist staff with focusing on problem areas unique for the patient’s treatment, behavior, or psychosocial well-being. Monitors and implements crisis intervention for those patients having active suicidal, homicidal or aggressive behavior because of diminishing mental and physical conditions. Implementation of one to one observation, medication administration, or rapid response. Provides goal/wrap-up groups and informal health education in the areas of medication compliance, suicide, smoking, and alcohol cessation while inpatient and upon discharge to patient and family members when applicable. She is competent in administering medications like insulin, and PRN effectiveness f/u within (60-120 minutes), monitors adverse reactions and critical syndromes (Extrapyramidal side effects, Tardive dyskinesia, and Neuroleptic malignant syndrome) associated with this class and use of antipsychotic medications for sustained quality clinical care and ...
