Tetralogy of Fallot with Pulmonary Atresia is an extreme form of tetralogy characterized by absence of flow from the right ventricle to the pulmonary arteries. Pulmonary blood flow is derived from a variety of sources, including native pulmonary artery branches and aorto-pulmonary collaterals with significant variability from patient to patient.

4. OBSERVATION
1. Major lobe of liver on right ,fundic gas on left ,straight
and short bronchus on right
2. Base apex axis of heart is 45 degree to left
3. Cardiac silhouette shows pulmonary bay empty
4. Prominent ascending aorta
5. Cardiac apex is off diaphragm
6. Oligemic lung fields
7. Reticular pattern in both the lung fields

5. Remark
 Tetralogy of Fallot with pulmonary atresia is the most
common possibility
