• Sign clearance form in the Dean’s Office
• Give form to Laura Anne or slide under HER office door –

• Make copies of all forms to keep in your professional
folder.

15. Criminal Background
Check
• Complete Criminal Background Check Form
• Give form to Laura Anne or Jean Edwards
• Complete on-line portion of CBC when prompted
• Complete county criminal background checks if
required
• Deadline – October 2, 2013
