L’idée de cette enquête nous est venue suite à la première Assemblée générale des référents AFFEP de 2014. Au moment du point à l’ordre du jour dévolu aux problématiques locales, nous nous sommes aperçus qu’un certain nombre de référents connaissait les mêmes difficultés dans leur subdivision lorsqu’un de leurs co-internes présentait des troubles psychiatriques ou psychologiques : personne ne savait véritablement comment agir, qui contacter, ou qui faire intervenir. Le coordinateur local n’était pas systématiquement informé, l’interne concerné était parfois mis en arrêt de travail, dans certains cas il était interdit de prescription ou d’entretien, quelques fois réorienté vers une autre spécialité. De façon quasi-systématique, l’association locale des internes et les co-internes de l’interne en souffrance étaient directement impliqués pour venir en aide à leur collègue et semblaient souvent désemparés. Et de façon étonnante, les dispositifs existants censés évaluer, aider, orienter les internes concernés n’étaient pas ou rarement sollicités ou effectifs (médecine du travail, médecine universitaire, médecin traitant, comités médicaux). Face à certaines situations à l’issue dramatique et pour éviter d’être à nouveau démunis, les internes référents AFFEP s’interrogeaient sur les dispositions à prendre dans ces situations. A l’issue de cette journée, cette question de la prise en charge des internes du DES de psychiatrie en souffrance nous est apparue essentielle à creuser, et nous avons donc voulu utiliser les ressources de l’AFFEP pour tenter d’y voir un peu plus clair...

2014 a finalement été une année faste pour l’AFFEP puisqu’il y a eu 2 enquêtes nationales : l’enquête Responsabilité
et cette enquête Internat et Post-Internat.
Replaçons-la dans son contexte : janvier 2014, le premier rapport Couraud/Pruvot sur la réforme du 3e cycle vient de
sortir et annonce une refonte totale de l’internat et possiblement du post-internat. Sonder les internes de psychiatrie
sur l’état de leur spécialité et sur son avenir semblait alors de mise.
Grâce aux résultats de cette enquête, nous avons pu présenter des chiffres forts, traduisant les souhaits de nos
membres pour leur formation et leur future pratique auprès des autorités de tutelles (ministères de la Santé et de
l’Education Nationale). Cela nous a permis de prendre une part tout à fait active au processus de rédaction du
cahier des charges de la future maquette du DES de Psychiatrie avec le CNUP.

Etude de la qualité des informations médicales des patients de 75 ans et plus admis aux urgences chirurgicales suite à une chute. Identification des facteurs de risque d’hospitalisation et de récidive de chute à 6 mois.

Un tiers des sujets âgés de 65 ans et plus chute au moins une fois par an, dont 15 % plusieurs fois par an. L’objectif principal de l’étude est de qualifier les informations recueillies par les soignants lors du passage aux urgences d’un sujet âgé suite à une chute. Les objectifs secondaires sont de quantifier le taux de rechute à 6 mois des patients rentrés immédiatement à domicile, et de déterminer les facteurs de risque de récidive.
L’étude a inclus les patients de 75 ans et plus admis aux urgences chirurgicales de Grenoble suite à une chute en novembre et décembre 2010. Les dossiers des urgences ont été analysés, puis les patients et leurs médecins traitants contactés 6 mois après la chute. Une analyse descriptive de la population totale et des sous-groupes (population initialement hospitalisée, population rentrée immédiatement à domicile, population ayant présenté au moins une chute dans les 6 mois, population n’ayant pas rechuté dans les 6 mois) a été réalisée.

Etat des lieux
Forte prévalence Européenne : 22 millions de femmes et 5,5 millions d’hommes1 concernés.
En France, augmentation attendue du nombre de fractures ostéoporotiques avec pour 2020 : +20.3 % fractures de hanche, +19 % fractures vertébrales et +17 % autres fractures.
Le coût des fractures ostéoporotiques, évalué à 32 milliards d’euros pour l’Europe en 2005, est estimé à 37 milliards d’euros pour 2025.
Malgré un enjeu de morbi-mortalité et économique certain, l’ostéoporose est un problème de santé négligé alors que des moyens diagnostiques et thérapeutiques efficaces existent.
Facteurs de risque d’ostéoporose d’intérêt
Âge > 60 ans, ménopause précoce avant 40 ans, antécédent personnel de fracture de faible traumatisme, antécédent familial de fracture du col au premier degré, corticothérapie au long cours, tabagisme actif ou sevré, faible apport calcique, carence en vitamine D, dénutrition, hyperthyroïdie évolutive non traitée, hypercorticisme, hyperparathyroïdie primitive non traitée, facteurs de risque de chute…

How Race, Age and Gender Shape Attitudes Towards Mental HealthThinkNow
Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).
