The My Conditions feature gives patient with chronic illnesses fast access to information they can use to monitor their condition, as well as provide information to their providers when requested (for example, by using linked patient-entered flowsheets for diabetic patients). This helps patients stay involved in the management of their chronic illness.You can identify patients as having a chronic illness using the following patient data: Diagnosis groupers Health maintenance modifiers Patient FYIsPatient Tour - My ConditionsPatients that have been identified as having a chronic illness by your facility have access to the My Conditions menu section for that illness, which appears above the other menu sections by default. If a patient clicks on the menu section, then Summary, he can view a Condition Summary report that contains useful information regarding his treatment and health status. The following items might appear: Medications he takes for his illness A list of his recent test results related to the illness A list of the Health Maintenance (preventive care) measures he can take to care for his illness My ConditionsEpic 2010Patient Clinical Information 133Patient-entered flowsheets he uses to provide his doctor with information about his glucose readings (for diabetic patients)The report contains hyperlinks so that he can open the Medications, Preventive Care, or Track Your Health pages.
The Preventive Care feature displays the health maintenance topics that are currently relevant to the patient.In this documentation, "Preventive Care" and "Health Maintenance" are synonymous. The reason for this is that these topics display to your providers in Hyperspace as "Health Maintenance topics," and they display to your patients in MyChart as "Preventive Care Services."
The Schedule an Appointment feature allows patients to select a slot for an appointment with their facilities. You can configure MyChart to use several visit types for scheduling. You can allow patients to schedule with different centers, and select different providers with whom to schedule appointments through MyChart.
You can add steps to a patient's direct scheduling workflow that ask the patient to verify demographics and insurance information. This helps you keep your patient records up-to-date with regards to demographics and insurance information. The information that is changed is only updated when the appointment is successfully scheduled.
Patients can initiate e-visits for specific, non-urgent reasons that your organization selects after they accept e-visit terms and conditions. If your organization chooses to include credit card payment for e-visits, patients first enter their credit card information. They then select an e-visit reason, the pharmacy where medications can be sent, and the physician who should receive the e-visit message.
Allow Patients to Request Updates to Their Charts Using MyChartPatients can request changes to their allergies, medications, and problems from the Allergies, Medications, and Current Health Issues Web pages in MyChart.
Patients can use MyChart to complete a health risk assessment online before they arrive at the clinic, as shown below. Clinicians then review the completed health risk assessments either from In Basket or within the Visit Navigator. If a patient has completed an assessment before, clinicians can compare past responses to the newest ones.
The Download Summary feature allows a patient to download a viewable or transferrable health summary file that includes information such as medications, allergies, problems, immunizations, procedures, and test results. The file can be saved as a Continuity of Care Document (CCD) that the patient can take and present for upload into another electronic health record or personal health record, such as Lucy.
