C/ “CRASH TEST” : ITALY
Together with my colleague Dr Bachir Athmani, we will be assessing a potential mass health / influx system every
day.
9,172 confirmed cases (7,375 the day before) and 463 deaths (almost 100 more than the day before)
Recall that 15% of the forms are severe and require hospital monitoring (venous route, O2, etc.); also, 5% of
infections require surveillance in intensive care, with discussion of VA (assisted ventilation) or even plasmapheresis
or "artificial lung" (staff of 6 people for 24/7) on often aged and polypathological sites.
Italy is in the acceleration phase since its first cases on February 1, with initial containment measures rather
limited (thus many cases “exported”: China, Algeria, etc.), but since yesterday has been going up a gear , with the
isolation of the North of the country (“economic lung”!), main source of infection.
In the North, doctors are already overwhelmed, particularly anesthesiologists

Avec mon collègue le Dr Bachir Athmani, nous évaluerons tous les jours, un système de sante / afflux massifs
potentiels de patients.
9172 cas confirmés (7375 veille) et 463 décès (soit près de 100 de plus que la veille)
Rappelons que 15% des formes sont sévères et nécessitent un monitoring hospitalier (voie veineuse, O2, etc);
également, 5% des infections nécessitent une surveillance en réanimation, avec discussion de VA (Ventilation
Assistée) voire plasmaphérèse ou « poumon artificiel » (staff de 6 personnes pour 24/7) sur des terrains souvent
âgés et polypathologiques.
L’Italie est en phase d'accélération depuis ses 1ers cas le 1er février, avec des mesures de confinement initiales
plutôt limitées (ainsi nombreux cas “exportés” : Chine, Algérie, etc), mais est depuis hier, passe a la vitesse
superieure, avec l`isolement du Nord du pays (“poumon économique” !), principal foyer d`infection.
Au Nord les médecins sont déjà débordés et particulièrement les anesthésistes

At the medical level: 2 strong axes:
1. Protect vulnerable people (essential): elderly people / EPHAD (limited visits), immunocompromised sites (chronic
pathology, iatrogeny, ...)
2. Protect the Hospital Health System: preparation of ICUs, masks, less scheduled surgery, etc.
Telemedicine is going to “explode” because we need doctors and uncontaminated doctors!
Limitation of “public” events with passage from the threshold number from 5,000 to 1,000 people (except 3 cases: events
(!), Transport, competition).
Schools: we can close them, not because the virus is dangerous for them (very rarely symptomatic) but above all because
children can easily transmit it (cf. contact with relatives) to vulnerable populations (> 65 years, pathology chronic can
decompensate, ...).

Au niveau médical : 2 axes forts :
1. Protéger les personnes vulnérables (essentiel) : personnes âgées / EPHAD (visites limitées), terrains immunodéprimés
(pathologie chronique, iatrogenie,...)
2. Protéger le système de Santé Hôpital : préparation des ICU, masques, moins de chirurgie programmée....
La télémédecine va “exploser” car on a besoin de médecins et de médecins non contaminés !
Limitation des évènements “publics” avec passage du nombre seuil de 5.000 à 1.000 personnes (sauf 3 cas :
manifestations (!), transport, concours).
Écoles : on peut les fermer, non pas parce que le virus est dangereux pour elles (très rarement symptomatique) mais
surtout parce que les enfants peuvent le transmettre facilement (cf. contacts avec les proches) à des populations
vulnérables (>65 ans, pathologie chronique pouvant décompenser,...).
