La plongée sous-marine en apnée ou en scaphandre connait un essor croissant ces dernières années, si en Algérie le nombre de licenciés est de plus de 5000, de ce fait les accidents en rapport apparaissent.
Parmi les accidents qu’on rencontre en plongée les accidents de désaturation restent rare en plongée loisir, mais occupent une place importante chez les plongeurs corailleurs de la région Est d’Algérie qui font des plongées hors normes et sans mesures de sécurité pour récolter le corail rouge.
L’accident de désaturation est une forme d’embolie gazeuse liée à un gaz inerte (N2) qui peut mettre en jeu le pronostic vital ou fonctionnel du plongeur

circulatoire, Rénale , Endocrine , Thermique
La Déshydratation, par augmentation de la diurèse est l’une des conséquences
majeures de l’immersion
3. Effets physiologiques de l’immersion
et conséquences sur le statut hydrominéral
plongeur s’immerge, la pression hydrostatique ambiante est pour l’essentiel
transmise aux tissus.
Redistribution du contenu sanguin des vaisseaux des membres vers le cœur droit
et s’accumule dans la circulation pulmonaire et le territoire vasculaire abdominal
(fig. 1).
Cette redistribution sanguine liée à l’effet compressif de l’immersion n’augmente
pas avec la profondeur, car l’écart de pression ne varie pas dans les membres entre
la pression interstitielle et péri vasculaire d’une part
D’autre part la pression veineuse proche de la pression atriale droite et largement
gouvernée par la pression des espaces gazeux pulmonaires.

5. Étape N° 1 : Augmentation du remplissage
cardiaque
Redistribution vasculaire, immédiate (« BloodShift » en langue anglaise) augmente
la pré charge cardiaque, le Volume d’éjection systolique (VES) et tend à augmenter
le débit cardiaque.
Simultanément : résistances vasculaires périphériques diminuent de l’ordre de 15 à
45% (inhibition de tonus sympathique + autorégulation vasculaire )
6. Étape N° 2 : adaptations précoces de
l’organisme (30minà120min)
1. Les modifications neuro-hormonales :
Hyper volémie relative
= qq secondes à qq minutes = > Distension des cavités cardiaques, des
veines caves et des artères pulmonaires stimule les barorécepteur
cardiopulmonaires à basse pression => Stim parasympathique
= Latence => Distension cavités cardiaques + hémodilution: Rénine,
aldosterone et ADH diminuent , libération du peptide natriurétique auriculaire
(PNA)
7. Sous l’effet de l’écart entre pression des espaces interstitiels et pression
capillaire dans les membres, un flux de liquide interstitiel vers le plasma s’instaure
=> augmente le volume plasmatique indépendamment de la redistribution
sanguine immédiate Cette augmentation de volume plasmatique contribue à la
stimulation de diurèse qui apparaît après15 à 30 min d’immersion.
2. Augmentation de la diurèse:
débit urinaire est le plus élevé pendant les deux premières heures et peut atteindre
6ml.min-1 , les plongeurs urinent entre 600ml et1500ml en fonction de la
température de l’eau.

3. La baisse du volume plasmatique:
Immersion induit en règle générale une hypovolémie iso-osmolaire => pas de
stimulation de la soif (sensation nécessaire à la correction des pertes hydriques )

10. Étape N° 3 : Les conséquences de ces
adaptations à la sortie de l’eau (fig. 3)
Dès que le sujet sort de l’eau, la gravité déplace le volume sanguin vers les
membres inférieurs.
La diminution du Volume Plasmatique qui était masquée par la réduction du lit
vasculaire lors de l’immersion, est donc brutalement démasquée. Elle est associée
à une baisse immédiate de la pré charge cardiaque
«Tilt Tests » réalisés dans les minutes suivant une immersion prolongée ont montré
qu’environ 50 % des plongeurs présentaient une hypotension orthostatique

12. Aspects Thermiques
Dans l’eau : pertes par convection + respiratoire (gaz détendu a faible humidité ou de
certains mélanges de gaz (hélium) présentant une meilleure conductivité)
vasoconstriction périphérique qui majore la redistribution du sang de la périphérie vers le
noyau central => diminution rapide des températures cutanées associée à une
augmentation légère et temporaire de la température centrale.
Activation des différentes réponses physiologiques( seuil et intensité) , sera différente d’un
plongeur à un autre , facteurs individuels (surface corporelle, pourcentage de masse
grasse, exposition quotidienne au froid, etc.), et de l’état physiologique avant et pendant
la plongée.
L’acclimatement va jouer un rôle important en modifiant les réponses physiologiques à la
contrainte thermique (vitesse de mise en œuvre des réponses, niveau des réponses,
adaptations morphologiques et métaboliques)
13. Hydratation
Avant : apport hydrique peut être considéré comme suffisant si l’urine est claire et
de couleur jaune pâle
Pendant : (Plongée très longues !!) L’ingestion de 200ml d’eau par heure
d’immersion semble faire consensus. La boisson ingérée doit contenir des glucides
pour maintenir une glycémie normale, et être légèrement hypo-osmotique (par
exemple,< 260 mOsm)
Après la plongée : boire un litre d’eau par heure, jusqu’à ce que les urines soient
claires ou pâles.
L’ingestion de boissons alcoolisées ou contenant de la caféine n’est pas appropriée
car augmentent artificiellement la production d’urine
la sensation de soif n’est jamais présente malgré une baisse majeure du
volume plasmatique.
