Eccentric training with or without core stabilization or stretching improved symptoms (61% improvement in the
Victorian Institute of Sports Assessment score, 95% confidence interval [CI] 53% to 69%). Surgery in patients
refractory to nonoperative treatment also improved symptoms (57%, 95% CI 52% to 62%) with similar outcomes
among arthroscopic and open approaches. Results from shockwave (54%, 95% CI 22% to 87%) and platelet-rich
plasma (PRP) studies (55%, 95% CI 5% to 105%) varied widely though PRP may accelerate early recovery. Finally,
steroid injection provided no benefit (20%, 95% CI −20% to 60%).
Conclusions :
1/Initial treatment of PT can consist of eccentric squat-
based therapy, shockwave, or PRP as monotherapy or
an adjunct to accelerate recovery
2/Surgery or shockwave can be considered for patients
who fail to improve after 6 months of conservative

PBMT increased the deposition of collagen type I and III in a gradual manner, with significant
differences relative to the group aged tendonitis (p < 0.001), and in relation to VEGF (p < 0.001);
decreased expression of MMP-3 and 9 were observed in group aged tendinopathy (p < 0.001).
PBMT, therefore, increased the production of collagen
type I and III, downregulated the expression of MMP-3 and MMP-9, and upregulated
that of VEGF, with age and age-induced hormonal deficiency.

Low-level laser therapy modulates pro-inflammatory cytokines

Our results showed that the pulsed LLLT seems to exert an anti-
inflammatory effect over injured tendons, with reduction of the
release of proinflammatory cytokines, such as TNF-α and the
decrease in the
i-NOS activity. Thanks to the pain reduction and the facilitation of
movement, there was a stimulation in the TGF-β and IL-1β release
In conclusion, we believe that pulsed LLLT worked
effectively as a therapy to reestablish the tendon
integrity after rupture

healing of Achilles tendon in rabbits.

The results for these parameters showed that PRP or LLLT alone has significant
advantages over untreated animals (P < 0.05). Furthermore, it was found that the
combined treatment with PRP and LLLT is even more efficient.
There was no significant difference (P > 0.05) between the two groups of LLLT and PRP.
However, the treatments combining PRP and LLLT showed significant results in
comparison of PRP or LLLT alone (P < 0.05). Our results demonstrate that the healing
time of injured tendon decreases by using the two therapies combined.

Add-On effect of LLLT + Cryotherapy (achille tendonitis)
CONCLUSIONS:
Our results demonstrate that cryotherapy in combination with
LLLT
can produce an anti-inflammatory "add-on" effect.
The order of therapy administration seems essential, as superior histology and
biomechanical results were found in the cryotherapy first/LLLT group.

The IL-1β + TNF-a dramatically induced the expressions of CCL2, CCL3, CCL5 and CCL20, as well as CXCL1, CXCL2, CXCL5 and
CXCL16, which were less decreased in HA group but strongly inhibited in PRP and HA + PRP groups
IL-1β + TNF-a worked as a positive feedback in chondrocytes which caused stronger activation of ILs family including IL1A, IL1B,
IL6, IL8, IL10, IL16, IL18 and ILRN in which HA showed no significant inhibition effect
However, the enhanced Ils expressions could be reduced by PRP, and highly declined by the HA + PRP treatment

HGF
Bendinelli P et al J Cell Physiol. 2010 Nov;225(3):757-66. doi:
10.1002/jcp.22274
*First, activated PRP in chondrocytes reduced the transactivating activity of NF-κB, critical regulator
of the inflammatory process, and decreased the expression of COX-2 and CXCR4 target genes
*By analyzing a panel of cytokines with different biological significance, in activated PRP we observed
increases in hepatocyte growth factor (HGF), interleukin-4 and tumor necrosis factor-α (TNF-α).
HGF and TNF-α, by disrupting NF-κB-transactivating activity, were important for the anti-inflammatory
function of activated PRP
*The key molecular mechanisms involved in PRP-inhibitory effects on NF-κB activity were for HGF the
enhanced cellular IkBα expression, that contributed to NF-κB-p65 subunit retention in the cytosol and
nucleo-cytoplasmic shuttling, and for TNF-α the p50/50 DNA-binding causing inhibition of target-gene
expression. Second, activated PRP in U937-monocytic cells reduced chemotaxis by inhibiting
chemokine transactivation and CXCR4-receptor expression, thus possibly controlling local
inflammation in cartilagePRP + HA against Bone Marrow Lesions with Bone Marrow
