Le chirurgien : quel bilan pré-opératoire ? (Tests hépatiques, vert d’indocyanine, volumétrie…) - Quels bilans avant Hépatectomie ?
Dr Nicolas GOLSE
Jeudi 30 janvier 2020
5. Introduction
•But du bilan ?
• Sélection
• Eviter l’insuffisance hépatique / décès
•Pas de recommandation sur bilan extra-hépatique
•Foie = multiples fonctions ➜ évaluation multiparamétrique
non standardisée
Transaminases
=
Reflet lésions cellulaires
PAS la fonction
9. Makuuchi et al. Semin Surg Oncol 1993
Clairance vert d’indocyanine
Attention si shunts intra-hépatiques,
modification débit hépatique, ictère
Reflet fonction globale du foie,
pas la fonction régionale (PVE, ALPPS…)
31. • N=31
• IHPH + décès: 6.5%
• Morbi-mortalité à 3 mois
• majorée si RLV-BWR ≤ 0.5%
• Non prédite par RLV-TLV Truant S et al. JACS. 2007
Ratio foie restant / poids corporel
32. Truant S et al. JACS. 2007
Ratio foie restant / poids corporel
33. Volume standardisé
• N=301
• Lésions primitives ou secondaires
• Standardized FLR (sFLR) = FLR volume (mL)/SLV (mL) x 100 (%)
• SLV = -794.41 + 1267.28 x body surface area
Kishi Y et al. Ann Surg. 2009
35. Fiabilité des estimations standardisées ?
Comparaison formules sTLV versus mTLV
Olthof P et al. Surgery 2019
50% patients ont
estimation < +/- 5% !...

38. TAKE HOME MESSAGES - VOLUMETRIE
1. Volumétrie indispensable avant Hx majeure
2. Foie sain: FLR/BW ≥ 0.5%
3. Chimio/cholestase/cirrhose: seuils non consensuels?
4. Place en Europe du volume standardisé ?
49. TAKE HOME MESSAGES
Imagerie contraste / scintigraphies
1. Faisceau d’études concordantes sur facteur
pronostique de l’imagerie DYNAMIQUE
2. Laquelle doit/va s’imposer ? Selon plateau à
disposition ?
3. IRM: « deux en un »
4. Place dans l’algorithme décisionnel vs tests bio ?
57. TAKE HOME MESSAGES
• Nombreuses fonctions imposent de nombreux tests (coût vs PHLF)
• Littérature abondante, surtout depuis 5-10 ans
• Biologie + imagerie statiques et dynamiques
• Endoscopie (VO) + radiologie interventionnelle (HVPG)
• Indices composites +++
• Utile même sur foie non cirrhotique !
• Pas de recommandation claire sur bilan « standardisable »
Bcp à dire
Très peu de reco
Très peu d’uniformisation des pratiques
défit d'être exhaustif et clair
MELD 2003
ICGR15 > 15% is a relevant, non-invasive and clearly accurate method to predict HD specially before minor hepatectomy.
n conclusion, we showed that high grade sinusoidal lesions occurred in most patients receiving OBC. High grade sinusoidal lesions lead to an increased risk of postoperative complications following major resection. These lesions could be predicted using simple scoring system such as the APRI score. This scoring system should be assessed prospectively as well as the implication of platelet in the occurrence of severe lesions should be investigated.
standardized FLR (sFLR) defined as FLR volume (mL)/SLV (mL) 100 (%). SLV was calculated by the formula: SLV 794.41 1267.28 body surface area (m2).
